Effective June 1, 2014, all children enrolling in kindergarten or transferring at any grade level from outside of Santa Clara County will be required to have their healthcare provider complete the Santa Clara County Public Health Department TB Risk Assessment for School Entry. This form must be completed by a licensed health professional in the United States within the last 12 months.
